1. Certified Passport Copy

Suitable certifiers: Town Hall Official (in France) e.g Mairie In Antibes, Accountant, Police Officer, Embassy or Bank Official, Notary/Lawyer
Further details on checklist in application pack or on web-site.
Must show address, contact number, name & job description of certifier

Certification in French acceptable from a French town hall, otherwise it must be in English. (Exact English wording on Lloyd’s web-site or checklist in application pack)

2. Letter from Captain addressed to Lloyds TSB Offshore on headed ships’ paper. A sample Captain's letter

It must confirm the following:

A. Your Job description e.g Stewardess, Engineer etc
B. Your monthly salary
C. Your home address. This can be anywhere worldwide on land. It cannot be the boat.
D. There must be a contact number for your captain on the letter. Lloyds will call him.
E. Name of employer and employer's address.
